Use this when a company has made cancelling harder than joining. It produces an unambiguous written cancellation that names the contract, states the effective date, and asks for written confirmation, so it cannot be quietly ignored or treated as an enquiry.

Common questions

Why does it ask for written confirmation?
Because a cancellation you cannot prove is the one that keeps billing you. Asking for confirmation, in writing, by a stated date gives you a record and a point at which to escalate. Keep the sent message too.
Do I have to give a reason?
No, and the reason field is optional. A reason can make the message read more naturally, but you are not obliged to justify leaving and you should expect a retention offer either way. If you do not want one, the message can simply state the cancellation.
What if they ignore it or say I have to call?
Send it anyway and keep the record, because written notice usually counts from the date you sent it regardless of what their process prefers. If they continue billing after your stated effective date, that becomes a billing dispute, and the Dispute a Bill Writer handles the next message.
Is making cancellation difficult even allowed?
Increasingly not. The FTC's click-to-cancel rule in the US requires cancellation to be as easy as signing up, and UK and EU consumer rules point the same way. If you are being sent in circles, saying so in writing and naming the regulator is legitimate.
